Alteplase is the name of the substance that destroys blood clots. It can be found in pharmacies under the name Actilyse and has no generic.
Alteplase is available as a powder for reconstitution in sodium chloride, glucose, or sterile water. This thrombolytic enzyme should not be used if the vial with the preparation powder is void.
Indications
Acute coronary occlusion (of thrombotic origin).

Bleeding.
Contraindications
Pregnancy risk C; sickle cell anemia; aneurysm; severe diabetes mellitus; for 2 months after intracerebral or intraspinal surgery; bacterial endocarditis; severe cerebrovascular problems; cerebral hemorrhage; severe internal bleeding (current or recent); severe liver disease; uncontrolled severe hypertension; arteriovenous malformation; acute pancreatitis; hemorrhagic problems; recent severe trauma or major surgery (depending on severity, up to 6 days after); hemorrhagic diabetic retinopathy; brain tumour.
How to use
Adults: 100 mg during the three hour period, being:
- 10 mg direct intravenous for 1 to 2 minutes;
- 50 mg for 60 minutes;
- 40 mg for 120 minutes.
Children: Efficacy and safety not established.
